Ergonomics is the applied science of equipment design, for the workplace, intended to maximize productivity by reducing operator fatigue, injury, and discomfort. Workspace assessments are scheduled upon request. Please answer the following questions to best inform the Occupational Safety team about your ergonomic needs.
For "Location", please indicate the workspace that you work in most often. You may add details, like your office number or suite, in the "Additional Location Details" box.
If you would like to upload any documentation (pictures, etc.), please feel free to do so in the "Add Supporting Documents" section. This Ergonomics Assessment is classified as an "incident report" type in CampusOptics, though it does not indicate that your ergonomic needs rise to the level of an incident as defined by workers' compensation or emergency management.
